Description

Summary:

Responsible as a member of the interdisciplinary team to the PCC and the attending physician for the provision of skilled nursing assessment, planning and care in order to maximize the comfort and health of patients and families consistent with RehabCare HomeCare and Hospice policies and procedures.Reports to the PCC.

Qualifications:

Education:

  • Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program, Bachelors of Science in Nursing, preferred.

    Licenses/Certification:

  • Valid RN license in the state employed. Current driver's license.

  • Current CPR certification. CHPCN certification, preferred.



Qualifications

Experience:

  • Minimum of 2 years experience as a professional nurse and preferably one year in an acute setting. Homecare or hospice experience, preferred.
