Position: Lead Case Manager / Medical Social Worker

Setting: Acute Rehab Unit

As a Lead Social Worker you will:

•CM/SW Supervisor Position

•RN Case Manager or Medical Social Worker

•Needs to have experience in medical social work/case management in an inpatient setting, preferably some experience in the post-acute setting (ARU/IRF experience would be best case scenario!)

•Needs to have strong leadership skills and embody our core values!

•Will still carry a small caseload (direct patient care)


Tasks:

·Lead and support the Inpatient Acute Rehab Unit Case Management/Social work team in daily operations

·Drive ARU performance improvement initiatives around patient safety, quality and experience

·Participate in leadership rounding and service excellence on the ARU

·Coordinate ARU CM/SW staffing

·Work cohesively with the ARU interdisciplinary team to ensure comprehensive and safe discharge planning

·Provide education and in-servicing to the ARU team around discharge planning and quality initiatives specific to case management

·Conduct audits and documentation review for compliance

·Complete performance reviews and competencies for the SW/CM team

·Manage new hire SW/CM onboarding, orientation and training

·Network with accepting facilities/agencies/vendors and build upon case management resources

Qualifications

As a Social Worker you will have:

  • Master’s Degree in Social Work from an accredited college or university
  • If required by state practice laws, current Social Work license (MSW) or eligibility
  • Experience in the provision of Social Service preferred in inpatient medical rehabilitation
  • Social Service related experience in a health care setting desirable, such as acute care and/or rehabilitation
  • Must have clinical experience working with individuals, families and groups.
  • Must have a working knowledge of basic services provided by various health and welfare organizations in the community
  • Current CPR certification, if required by the facility assigned to.
  • Knowledge and skills necessary to provide services appropriate to the age of the residents served. 
  • Computer skills and knowledge of basic computer program
  • Must have strong leadership skills
