Job Details


 The RN shall be responsible for providing a full range of nursing care including assessment, treatment planning and hands-on nursing care in an homecare environment. The RN must demonstrate accountability and contribution to care planning development, quality improvement, problem-solving and productivity enhancement.

Essential Duties:

  • Adheres to all applicable policies/procedures and practice guidelines.
  • Provides clinically appropriate therapeutic interventions.
  • Documents patient/resident/client care and response to treatment and provides feedback to supervising therapist, as indicated.
  • Communicates changes in patient/resident/client status to the supervising nurse.
  • Provides patient/resident/client and family/caregiver education and training that assist’s with continued functional independence.
  • Coordinates treatment with other disciplines.



  • Must be a graduate if an accredited RN nursing program
  • Must have a current state license to practice
  • One year of home care experience

Physical Requirements:

  • Ability to lift/transfer patients and equipment.
  • Frequent and prolonged bending, stooping, crouching, walking, and standing.
