Texas Rehabilitation Hospital Keller is located in beautiful Keller, TX that is a joint venture between Texas Health Resource and Kindred Healthcare.  It is a 36-bed rehabilitation hospital serve patients recovering from strokes and other long-term illnesses or injuries and feature specialized clinical programs.

Texas Rehabilitation Hospital Keller's mission is to deliver comprehensive, efficient medical rehabilitation to persons with disabling conditions in order to provide them the opportunity to achieve the highest attainable level of functional improvement. Specialty programs include stroke rehabilitation, brain injury rehab, orthopedic physical therapy, and amputation treatment.

The Outcomes Manager coordinates timely and accurate data completion of the Inpatient Rehabilitation Facility- Patient Assessment Instrument (IRF-PAI) for each rehabilitation inpatient.

Reviews and monitors the information contained in the IRF-PAI, on each patient to assure accuracy in the scoring of functional status and reflection of the patient’s care needs as documented in UDS-Pro.

Responsible for staff orientation and ongoing education and facilitate biannual UDS MR staff credentialing for QI scoring.

• Graduation from an accredited nursing program,  BNS, Associate Degree in Nursing or Nursing Diploma program
• Current unresticed LPN/RN licensure preferred, or current PT, OT, SLP
• CRRN preferred
• CPR/BCLS certification.
• UDSMR PPS Certification within 3 years.
• 3 to 5 years of experience in the care of patients in the acute hospital or rehabilitation setting preferred.
