Position: RN Case Manager

Setting: Acute Rehab

FTE: Full-time

City / State: East Chicago, IN 

QUALIFICATIONS: The incumbent shall, at a minimum, be a healthcare professional with 3-5 years clinical experience preferably in med-surg and post acute care settings. Incumbent must hold Masters in Social Work as required by state (VERIFY STATE REQUIRMENT). Registered Nurse strongly preferred. Other licensed professional or certified case manager acceptable. Incumbent shall have demonstrated progressive responsibility in previous job roles, positive communication skills and basic computer word processing ability. Knowledge of reimbursement benefits and requirements including but not limited to Medicare, Medicaid and managed care is required. Incumbent must have computer and current technology competence. Must have good organizational skills, time management skills and analytical ability in order to interpret information and carry out duties independently.

STATEMENT: The incumbent shall be responsible for providing care coordination including needs assessment and identification of care options, communication with patients and families in an interdisciplinary environment consistent with the position's qualifications, professional practices and ethical standards. The incumbent gathers and manages patient data related to case management, care planning, and QI analysis. The incumbent shall also demonstrate accountability for and contribution to program development, quality improvement, problem solving, and productivity enhancement in a flexible interdisciplinary fashion. The incumbent shall be accountable for carrying out all responsibilities in accordance with Kindred Healthcare CORE values..
